As the coach and or player, we value your opinion as to the performance of the Referees and Assistant Referees that have refereed your games. Do not use this form just for complaints or problems; we would like to hear about the good jobs that are done also. The New York West Referee Association Board and/or the State Referee Administration will address any comments about improper behavior by a Referee. Positive comments may help us in deciding out Referee of the Year.
